High over the Indonesian islands, an Airbus A320 hums along on autopilot at cruising height.

In the cockpit, both seats are still.

Air traffic control calls the flight, waits, and gets nothing back.

It calls again, and again there is no answer.

Behind the two control sticks, both pilots are fast asleep, and the plane is drifting off its route.

How do two trained pilots, both cleared as fit to fly that morning, end up asleep at the same time?

Why a fitness check cannot see a sleepless night

A pre flight fitness check looks at the body, not the night before. It reads heart rate, blood pressure, eyesight and illness, and by every one of those measures both men were perfectly fit.

None of it measures sleep.

There is no quick test for a wrecked night, so a pilot can pass every check and still climb into the seat running on almost nothing.

The co-pilot was healthy and badly short of sleep, worn down by month old twins at home and a house move, and he did not flag how tired he really was.

That is the gap the checks leave open. Fitness and rest are not the same thing, and a system that measures the first while trusting a tired man to report the second will sometimes put two exhausted pilots in the same cockpit.

The 28 minutes no one answered

At cruise the captain asked to rest his eyes and handed the controls across. The co-pilot waved off a rest of his own, meant to keep watch, and then dozed off too.

The plane flew on without them.

Its autopilot held the last heading faithfully, but a held heading is not the assigned route, so the A320 slid steadily away from where it should have been while controllers called with no reply. For 28 minutes the radio went unanswered.

Then a call finally landed. The co-pilot woke, answered, and the crew turned the jet back onto course and flew on to land with no one hurt, a near miss measured in minutes rather than feet.

What the investigators pieced together

The numbers came out in the report. The flight was a Batik Air A320 running from Kendari to Jakarta, carrying 153 passengers and six crew, 159 people in all, cruising at around 36,000 feet when both pilots went under.

The rest history was the heart of it.

The co-pilot had about 53 hours off before the trip, but much of it went on the newborn twins and the move, so the clock said rested while the man was not. Indonesia’s transport safety committee traced the whole event back to unmanaged fatigue.

The paperwork had a hole in it. The airline told crews to run a cockpit check every 30 minutes, yet never said whose job it was, so an exhausted pilot alone at the controls was left to police himself.

What kept it from becoming a disaster

It helps to be clear about what did not happen. The jet did not stall, dive or drop out of the sky, because the autopilot flew it steadily the entire time the two men slept.

The danger was subtler than a crash.

A plane off its route and out of radio contact in busy airspace is a real hazard even while it flies straight and level, which is why the near miss rattled regulators, much as a fox has to relearn a field once it fills with solar panels. The risk was in the silence, not the machine.

And it was one flight. Two pilots nodding off on the same leg looks like a freak event, but investigators read it as a system problem, the predictable end of rest rules that leaned on tired people to flag themselves.

What changed after the cockpit went silent

The report did not stop at blame. It pushed for real fatigue management, a proper rest routine before a flight and firmer rules for the night and early hours when the body most wants to sleep, the same human factor that shadows an offshore wind crew.

The harder part has no checklist.

You can measure a pilot’s blood pressure, but you cannot fully measure the pull of a bad week and a new baby, which is why the safety board leaned on better procedures rather than a new test. Rest has to be built into the schedule, not left to a tired man’s honesty.

The unsettling part is how ordinary it was. Two healthy, certified pilots, a routine cruise, a working plane, and for 28 minutes no one flying it, which is exactly why the fix has to reach the whole system, not just the two who fell asleep.
